What the Plan Covers:

• 2 cleanings per year
• 1 set of Bitewing X-rays
• 2 Comprehensive Exams
• 2 Fluoride Treatments
• 1 Emergency Exam
• 15% off of all services rendered within the 12 months.
• 20% off of Veneers, Sleep Apnea Appliances, and 6-Month Smiles adult orthodontics

What the Plan Costs:

• $505 per family member
• $485 per family member if 3 or more join
• $485 per member for small business plans

Other Benefits of the Plan:

• No preauthorization required
• No waiting periods or exclusions for pre-existing conditions
• No deductible
• No age limit
• Spend your dollars on whatever service you choose (ie teeth whitening)

Terms and Limitations of the Plan:

• Family members cannot be substituted in for another family member.
• Family members can be added at any time during the year. However, the end coverage date will remain the same for all members.
• Cleanings must be done before the end coverage date.
• Co-payment is due at time of service for any work received.
• Rates are subject to change.
